We are reaching out to you with a survey designed within the scope of the European Network of Youth Cancer Survivors. The network is part of EU-CAYAS-NET, a project co-funded by the European Union with a mission to improve the lives of young people who had cancer. Read more about this project here.

The main aim of this survey is to gain knowledge about current existing LTFU-care for childhood, adolescent and young adult cancer survivors within Europe and identify best practices, facilitators and barriers in those areas. To get the most comprehensive picture possible, the input of health care professionals who work in long-term follow up-care and/or childhood, adolescent or young adult survivors are valuable to us. It would therefore be much appreciated if you want to share your experiences and views by filling out this survey.

With your help, we want to:

1. Create a European map which visualizes where LTFU care is available

2. Publish a report with best practices and gaps on relevant topics concerning LTFU care

3. Make recommendations for and promote the importance of appropriate and effective LTFU care across Europe
